Profilo professionale e clinico
In qualità di Professore di Neurologia, il Prof. Massacesi svolge principalmente attività di ricerca e didattica. La sua attività di ricerca è focalizzata sulle malattie infiammatorie del sistema nervoso (come per esempio la sclerosi multipla) e sulla traslazione clinica dei risultati ottenuti. È specialista in Neurologia dal 1986 e in Immunologia Clinica dal 1990.
Come Direttore di uno dei reparti di Neurologia dell’AOU Careggi, è responsabile della diagnosi e del trattamento di tutte le patologie neurologiche dell’adulto. Coordina inoltre l’ambulatorio per migliaia di pazienti cronici affetti da malattie infiammatorie del sistema nervoso centrale e periferico, come sclerosi multipla (SM), vasculiti croniche del SNC, encefaliti, neuropatie infiammatorie periferiche, sindromi miasteniche.
In qualità di Coordinatore dell’Area Ricerca dell’AOU Careggi, supervisiona i programmi di supporto alla ricerca clinica dell’ospedale.

LUCA MASSACESI, PROFESSIONAL BIOGRAPHYCurrent positionsDr. Luca Massacesi, MD, has been and currently is a full professor of Neurology at the Department of Neurosciences of the University of Florence since 2005 and Director of the Department of Urgent Neurology (formerly Neurology 2) of the Careggi University Hospital, since 2008, in Florence, Italy. In addition Prof. Massacesi Coordinates the Research Area of the Careggi Univeristy Hospital in Florence.
Clinical Professional SummaryAs Professor of Neurology od the University of Florence the main activity of Prof Massacesi are research and teaching. As for research the main focus is on inflammatory diseases of the nervous system (as for instance multiple sclerosis)and translation to patients of the results of this activity. Prof Massacesi however - also board-certified in Neurology since 1986 and in Clinical Immunology since 1990 -, has relevant clinical duties as the Chairman of one of the Neurology Departments of the Careggi University Hospital and as such he is in charge of diagnosis and treatment of any neurological disease of adult peple. In addition - through the outpatient clinic - he is also in charge of thousands of chronic patients with inflammatory diseases of the nervous system, both peripheral and central, as multiple sclerosis (MS) and the related disorders, chronic vasculitis of the nervous system (CNS), encephalitis, inflammatory peripheral neuropaties, miastenic syndromes. In addition - as Coordinator of the recently established Research Area of the Careggi University Hospital, Prof. Massacesi is in charge of coordinating the support programs for clinical research in this Hospital
Reasearch anc Professional milestones- Animal model of multiple sclerosis. Prof. Massacesi late in the ‘80es spent his post-doctoral fellowship at the Massachusetts General Hospital and Harvard University (Boston Ma, USA) late in the eighties, critically contributing to the development and the immunological characterization of new animal models of multiple sclerosis (MS) as the experimental autoimmune encephalomyelitis (EAE) in non-human primates, now internationally renowned as a reference in the field. - The European network on T-cell autoimmunity in multiple sclerosis. After his postdoctoral fellowship he was hired by by the Florence University and the Careggi University Hospital. Applying from this Intitutions he became the Project Leader of the “European Concerted Action on T-cell autoimmunity in Multiple Sclerosis”, a multicentric study funded by the R&D program of the Directorate General XII of the European Union Commission (by a program equivalent to the current European Research Council grants. Beside coordination of the network, Prof. Massacesi was in charge of the EAE model that was impemented in a Duch primate center, that allowed the development of fundamental hypotheses on the pathogenic mechanisms of MS (role of B as well as of T lymphocytes in the effector mechanisms of brain lesion development; histological basis and the kinetics of the brain MS lesion formation that later on led to the discovery of the central vein sign (CVS), the MRI marker specific of MS; c the compartmentalization of immune response in the CNS in the course of MS chronic brain lesions.o B and T lymphocytes in MS pathogenesis. The role of B lymhocytes and their relation with T lymphocytes in the effector mechanisms leading to the MS brain lesion development, that led to the discovery of the efficacy of CD20+B-cell depletion in the therapy of MS was characterized by his Mentor professor Stephen Hauser in marmoset EAE, the nineties.o The Central Vein Sign. after in 2009 Prof. Massacesi spent one sabbatical year at the Neuroimmunology Branch of the National Institutes of Helth (Bethesda, Md, USA), Prof. Massacesi indeed remarkably contributed to the characterization and validation of the CVS, by a project led Dr. Daniel Reich, chief of the Translational Neuroradiology Branch of the same institute - This marker is now remarkably improving the specificity of the otherwise poorly specific MS diagnostic criteria.o Compartmentalization of the immune response in the CNS during the course of MS. The data generated in the EAE indudced in non-humen primates, provided the experimental data that supported the development of the pathogenic hypothesis that chronic progression of MS disability is based on the compartmentalization and chronicization of inflammatory lesions in the CNS. This hypothesis eventually led to the discovery of one critical MRI marker of this mechanism - the Paramagnetic Rim Lesions (PRL) - and to the related comprehension of how disability can progress independently from relapses - Contribution to neuroimmunology. The broadly recognized interdisciplinary clinical expertise of Prof. Massacesi, in the last decades, also contributed to the development of basic and clinical neuroimmunology as a new independent field, eventually renowned as one of the most innovative subspecialties of the whole neurology. In many neurological scientific societies, this understanding led to the development of some of the most participated study groups, as in the Italian Society of Neurology (SIN) and in the European Academy of Neurology (EAN). Both of these groups have been chaired by Prof Massacesi. - Clinical trials. Over the last two decades, the scientific interests of Professor Massacesi drifted from translational to mainly clinical, leading a large number of independent studies. Among these a multicentric independent Italian RCT, funded by the Italian Medicine Agency (AIFA) that proved the equivalent efficacy in MS of an old inexpensive immunosuppressive agent (azathioprine) vs interferon beta.- Medicinal drugs regulatory experience. For his original and remarkable expertise in clinical trials coupled to his independence from private interests on the field, during the last two decades, Prof. Massacesi was appointed by the European Medicines Agency (EMA) as core member of the Scientific Advisory Group (SAG-) Neurology and up to a few years ago, he was also a consultant for neurological medications of AIFA. These appointments allowed Professor Massacesi to consult EMA on Marketing Authorizations and AIFA on reimbursement for tens of medicines for neurological diseases, thus developing a unique record on medicinal drugs marketing authorization at the European level and on reimbursement authorization at the national level.
Publishing and editorial activity. The above-mentioned achievements generated about 250 publications by Prof. Massacesi, the vast majority of them in international peer-reviewed scientific journals. In addition, prof Massacesi is part of the Editorial Board of two international scientific journals specialized in the neurology field.
Future perspectives.The long academic and consulting career of Professor Massacesi, besides the above-described scientific achievements, was characterized by international renown, independence from private interests as proven by the number of independent clinical trials, mainly in MS. The large number of sponsored lectures and consultations for private entities made in the past years have always been conducted, fulfilling the strict requirements on conflict of interest prevention of the University of Florence and of the Italian laws and of the above-mentioned regulatory agencies.
